Volunteer Registration
All SMMA volunteers are required to be finger printed and have background checks run by MHUSD. Use this volunteer registration form to apply and register. Once you are cleared you will be issued a photo ID by the district.
Reimbursement Request Form
You can use this form to request a reimbursement check if you have been approved to make a purchase on behalf of SMMA using your own funds.
